Empower: Design of a digital intervention for workplace stress and mental health. A European study

Introduction
Work stress, anxiety and depression have an enormous impact on the well-being of employees, their employers, and society. Due to the loss of productivity, common mental disorders have a substantial economic impact. Major depression alone has been attributed to 50% of long-term absences from work, and depressive symptoms are related to lowered productivity while at work. Anxiety also contributes to loss of productivity and sickness absence. Treatment of common mental disorders in a work setting may improve symptoms, however, that does not automatically lead to improved work productivity. Addressing mental well-being at the workplace might improve work functioning, and digital interventions have been introduced with that objective. However, their evaluation in research has been limited.
The European Intervention to Promote Wellbeing and Health in the Workplace (EMPOWER) digital intervention is designed to provide and evaluate an integrative user programme that meets the needs of employees and employers in addressing work stress.
This work was supported by the European Union Horizon 2020 Research and Innovation Programme Health (grant number APP1195937, 848180). The EMPOWER project started 1.1.2020 and is currently ongoing.
ObjectivesWe aim to
1) describe the design and development of the digital intervention.
2) culturally validate the intervention in three countries
3) test the prototype and beta version for its usability in the RCT to evaluate its effect in four countries that is currently ongoing.
MethodsA user-centred design process was followed from January 2020 until November 2021 to create a beta version for usability testing. A tailored algorithm was developed to provide support at the individual employee level and the company level. Each element of the digital intervention was translated and culturally validated in four languages in Spain, the United Kingdom, Poland, and Finland. Usability testing was conducted in each country (n=31) to explore validity, usability, and user experience.
ResultsThe digital intervention consists of a website and a mobile application (app). The website has a public section and an employer portal that provides recommendations to reduce psychosocial risks in their company based upon clustered input from employees. The app provides algorithm-based personalised content after assessing a user’s physical and psychological symptoms, work functioning, and psychosocial risk factors for work stress. The usability testing improved the flow through the app and high ease of use and completion of tasks by participants.
ConclusionsThe EMPOWER digital intervention is a tailored multimodal intervention addressing wellbeing, work stress, mental and physical health problems, and work productivity. Usability testing provided validation of the app as version to be evaluated in the EMPOWER RCT.

Introduction
Intramuscular injections (IMI) remain a frequent practice in mental health. The available guidelines for IMI in mental health only focus on the technical side of the practices. Moreover, no recent update has been performed to improve practice of IMI in mental health
ObjectivesTo assess a formalized consensus agreement regarding the best practice concerning IMI in mental health and to develop practice guidelines.
MethodsA two-round Delphi method was used. The scientific committee consisted in one psychiatrist, one orthopaedic surgeon, one infection control practitioner, one hospital pharmacist, one mental health nurse, one nurse exploring care relationship and one nurse educator. From literature review, each expert proposed specific recommendations. The panel experts were asked to rate the appropriateness and the applicability in current practice of each recommendation on a 9-point Likert scale. Panel members were recruited in five mental health institutions. The first round questionnaire was emailed to each respondent on February 2020 and the second one on June. Propositions were considered appropriate and applicable in current practice if the median was >=7. Agreement among experts were judged by the statistical measure of the Interpercentile Range
ResultsFrom the first round, 46 recommendations were retained by 49 nurses. 27 propositions were retained after this second round by 32 nurses. The scientific committee added 12 other recommendations because of their importance in the literature and clinical practice.
ConclusionsThis study provides consensus-based recommendations on IMI in mental health. Nursing staff need to be educated about the new guidelines from both the theoretical and clinical perspectives

Long-Term Mental Health Effects of a Devastating Wildfire Are Amplified by Sociodemographic and Clinical Antecedents in College Students

Objectives:
The aim of this study is to assess prevalence of major depressive disorder (MDD), generalized anxiety disorder (GAD), and posttraumatic stress disorder (PTSD) in students of Keyano College 18 months after a wildfire and to determine the predictors of likely MDD, GAD, and PTSD in the respondents.
Methods:A quantitative cross-sectional survey was used to collect data through self-administered, paper-based questionnaires to determine likely MDD, GAD, and PTSD using the PHQ 9, GAD-7, and the PTSD Checklist for DSM 5, Part 3, respectively. Data were analyzed with SPSS version 20 (IBM Corp, Armonk, NY) using univariate analysis with chi-square tests.
Results:Eighteen months after the wildfire, the 1-month prevalence rates for MDD, GAD, and PTSD among the college students were 23.4%, 18.7%, and 11.0%, respectively. There were statistically significant associations between multiple sociodemographic variables and the likelihood respondents presented with MDD, GAD, and PTSD 18 months after the wildfire. There were also associations between the likely MDD, GAD, and PTSD and abuse/dependence on alcohol and substances in respondents at 18 months.
Conclusion:Our study has established prevalence rates for MDD, GAD, and PTDS among college students 18 months after the Fort McMurray wildfires. Further studies are needed to explore the impact of college-based mental health interventions on the long-term mental health effects of the wildfires.
Effects of TiO2 Blocking Layer on Photovoltaic Characteristics of TiO2/Nb2O5 Dye Sensitized Solar Cells

This study reports on the effect of introducing TiO2 compact layer on the photovoltaic characteristics of TiO2/Nb2O5 composite dye sensitized solar cell. The compact layer was deposited by spray pyrolysis technique. It was observed that introduction of 60 nm thick compact layer improved the short circuit current density Jsc ,Open circuit voltage Voc, and efficiency of the cell from 4.9 mA/cm2 to 8.2 mA/cm2, 6.8×10-1 V to 7.2×10-1 V and 1.9 % to 3.4 % respectively compared to traditional cell prepared without compact layer. Electrochemical impedance spectroscopy confirmed an increase in recombination resistance from 5.5×101 Ω.cm2 for bare DSSC to 9.0×101 Ω.cm2 for DSSC with compact layer thereby improving electron lifetime of the cells from 2.5×10-4 s to 386.9×10-4 s.
Application of An Organic Plant-Derived Binder in the Fabrication of Diatomaceous Earth Waste-Based Membranes for Water Purification Systems

This work reports on the use of diatomaceous earth (DE) waste and organic binder derived from Corchorus olitorius, locally known as “Mrenda” in the design of an efficient water filtration membranes. Charcoal powder was incorporated to enhance the porosity of the membrane. The firing was done at temperatures varying from 700.0 °C to 1150.0 °C. The DE waste samples comprised 79.0% silica (by mass) and 11.0% total flux content compared to porter’s clay that had 50.0% silica, 28.8% AL2O3 and 7.0% total flux content. On the other hand, the “Mrenda” binder contained 6.5% total organic matter. The use of the plant-derived binder enhanced the mechanical strength of the greenware by 52.7% and the fired membranes by 152.2%. The fabricated DE waste-based membranes were 15.0% stronger than clay-based ceramic membranes prepared under similar conditions. A sintering temperature of 900.0 °C was optimal in producing porous membranes for filtering of 4.1 liters of water per hour. The pore diameter of the membranes fabricated from DE waste only ranged between 2.0 nm – 99.0 nm. On micro-organisms filtering efficacy, the DE waste-based membranes and those fabricated with 5.0% charcoal were 99.9% and 88.4% effective in the removal of E. coli and Rotavirus respectively.
Postconcussive Symptoms Following Mild TBI and Extracranial Injury: What Are the Contributing Factors?

Objectives:
Whether mild traumatic brain injury (mTBI) sustained by children results in persistent or recurrent symptoms, over and above those experienced by children who solely sustain mild extracranial injuries, remains debated. The current prospective longitudinal case-controlled study aimed to examine the relative influence of injury and noninjury factors on symptoms in preschool and primary school-aged children who sustained an mTBI or mild extracranial injury at least 8 month earlier.
Methods:Participants were 64 parents of children (31 mTBI, 33 trauma controls) who sustained injury between ages 2 and 12, whose postconcussive symptoms across the first 3-month postinjury have been previously described. The current study assessed postconcussive symptoms at 8 or more months postinjury (M = 24.3, SD = 8.4) and examined a range of injury and noninjury predictive factors.
Results:At or beyond 8-month postinjury, symptom numbers in the mTBI group were comparable with those of the group who sustained mild extracranial injury. Educational attainment of parents (below or above high-school attainment level) was the only predictor of symptoms at follow-up, with preexisting learning difficulties approaching significance as a predictor.
Conclusions:While our earlier study found that mTBI was associated with symptoms at 3-month postinjury, follow-up at more than 8 months showed mTBI no longer predicted symptom reporting. While mTBI contributes significantly to the presence of symptoms in the first few months postinjury, researchers and healthcare practitioners in this field need to consider the potential impact of noninjury factors on persistent or recurrent symptoms after mTBI.
Predictors of Post-concussive Symptoms in Young Children: Injury versus Non-injury Related Factors

Objectives: A notable minority of children will experience persistent post-concussive symptoms (PCS) following mild traumatic brain injury (mTBI), likely maintained by a combination of injury and non-injury related factors. Adopting a prospective longitudinal design, this study aimed to investigate the relative influence of child, family, and injury factors on both acute and persistent PCS in young children. Methods: Participants were 101 children aged 2–12 who presented to an Emergency Department, with either mTBI or minor bodily trauma (control). PCS were assessed at time of injury, 1 week, and 1, 2, and 3 months post-injury. Predictors included injury and demographic variables, premorbid child behavior, sleep hygiene, and parental stress. Random effects ordinal logistic regression models were used to analyze the relative influence of these predictors on PCS at early (acute – 1 week) and late (1–3 month) post-injury phases. Results: Presence of mTBI was a stronger predictor of PCS in the early [odds ratio (OR)=18.2] compared with late (OR=7.3) post-injury phase. Older age at injury and pre-existing learning difficulties were significant predictors of PCS beyond 1 month post-injury. Family factors, including higher levels of parental stress, higher socio-economic status, and being of Anglo-Saxon descent, consistently predicted greater PCS. Conclusions: Injury characteristics were significantly associated with PCS for 3 months following mTBI but the association weakened over time. On the other hand, pre-existing child and family factors displayed an increasingly strong association with PCS over time. Follow-up for these “at-risk” children which also addresses family stress may minimize longer-term complications. (JINS, 2016, 22, 793–803)

We analyze the dust emission features seen in Spitzer Space Telescope Infrared Spectrograph (IRS) spectra of red supergiant (RSG) and oxygen-rich asymptotic giant branch (AGB) stars in the Large Magellanic Cloud and Small Magellanic Cloud galaxies and in various Milky Way globular clusters. The spectra come from the Spitzer Legacy program SAGE-Spectroscopy (PI: F. Kemper), the Spitzer program SMC-Spec (PI: G. Sloan), and other archival Spitzer-IRS programs. The broad 10 and 20 micron emission features attributed to amorphous dust of silicate composition seen in the spectra show evidence for systematic differences in the centroid of both emission features between O-rich AGB and RSG populations. Radiative transfer modeling using the GRAMS grid of models of AGB and RSG stars suggests that the centroid differences are due to differences in dust properties. We investigate differences in dust composition, size, shape, etc that might be responsible for these spectral differences. We explore how these differences may arise from the different circumstellar environments around RSG and O-rich AGB stars and assess effects of varying metallicity (LMC versus SMC versus Milky Way globular cluster) and other properties (mass-loss rate, luminosity, etc.) on the dust originating from these stars. The U.S. broiler industry is highly vertically integrated and increasingly concentrated in the number of firms and production areas. These structural elements could have implications for performance and the functioning of the law of one price (LOP) across regions. This article investigates this using data on four regional markets. Cointegration results indicate that regional prices are spatially linked in the long run, but pairwise cointegration was not found, suggesting that the LOP does not hold. Causality tests confirm the relative importance of price shocks from the South. This finding is reflective of price coordination by firms with production in multiple regions.

Since 1991, in France, studies on the conditioning of iodine were carried out to assess the potential of several specific inorganic host matrices. The apatite family has been mainly studied because of its good chemical durability and its ability to confine iodine over geological time scales. A lead-bearing apatite, Pb10(VO4)4.8(PO4)1.2I2, and a calcium-bearing apatite, Ca10(PO4)6(OH)2-x(IO3)x, were selected on the basis of their incorporation rate (between 7 and 10 wt.%) and a satisfactory resistance to leaching (V0(50 °C, pure water) ∼ 10-2 g.m-2.d-1; Vr(50 °C, pure water) < 10-4 g.m-2.d-1). However, with such materials, the removal of open porosity requires non conventional sintering techniques like spark plasma sintering to decrease the surface exposed to water. This is why, in parallel, other matrices, like silver phosphate glasses, have also been investigated. To improve the chemical durability and thermal properties of these glasses, cross-linking reagents were added to their formulation.

Clustering commodity displays into a Tiled Display Wall (TDW) provides a cost-effective way to create an extremely high resolution display, capable of approaching the image sizes now generated by modern astronomical instruments. Many research institutions have constructed TDWs on the basis that they will improve the scientific outcomes of astronomical imagery. We test this concept by presenting sample images to astronomers and non-astronomers using a standard desktop display (SDD) and a TDW. These samples include standard English words, wide field galaxy surveys and nebulae mosaics from the Hubble telescope. Our experiments show that TDWs provide a better environment than SDDs for searching for small targets in large images. They also show that astronomers tend to be better at searching images for targets than non-astronomers, both groups are generally better when employing physical navigation as opposed to virtual navigation, and that the combination of two non-astronomers using a TDW rivals the experience of a single astronomer. However, there is also a large distribution in aptitude amongst the participants and the nature of the content also plays a significant role in success.

Summary
This book chapter is dedicated to Peter Safar, the father of modern resuscitation, and world leading pioneer in the field of therapeutic hypothermia.
Introduction
The history of induced hypothermia began in the 1950s with elective moderate hypothermia of the brain, introduced under anesthesia, for the protection–preservation during brain ischemia needed for surgery on heart or brain. In the early 1960s, Peter Safar recommended the use of therapeutic resuscitative hypothermia for humans after cardiac arrest in his cardiopulmonary–cerebral resuscitation algorithm. At this time, it was thought that moderate hypothermia (28–32 °C) was required for brain protection. Resuscitative hypothermia research was then given up for 25 years, as experimental and clinical trials had been complicated by the injurious systemic effects of total body cooling, such as shivering, vasospasm, increased plasma viscosity, increased hematocrit, hypocoagulation, arrhythmias, and ventricular fibrillation, when temperatures dropped below 30 °C, and lowered resistance to infection during prolonged moderate hypothermia. Moderate hypothermia was too difficult to induce and to maintain.
Peter Safar deserves most of the credit that mild therapeutic hypothermia was re-discovered in the mid 1980s. When he considered the reasons for various outcomes with the same durations of cardiac arrest in his dog experiments, he observed that relatively small differences in brain temperature in the range of mild hypothermia (33–36 °C) at the start of the experiments had a major influence on neurologic outcome. He and his research group then confirmed these observations in systematic studies of mild hypothermia before, during, and after cardiac arrest in dogs.

Some microalga species may accumulate extracellular polysaccharides, such as a gelatinous mass that encloses its cells, and is known as envelopes, sheaths or capsules. These structures are very common among the Desmidiales. However, studies concerning their functions, properties and structures are still incipient. Spondylosium panduriforme is a filamentous desmids, enclosed by a great and continuous capsule with well-defined borders firmly bond to the cell. These characteristics can be observed through light microscopy in Indian ink preparations. The S. panduriforme capsule is taken as an essential part of the cell, and points out the importance of this structure for its proper functioning [1,2].

Administraation of antiretroviral therapy to women during pregnancy, labor and delivery, and to infants postnatally can dramatidy reduce mother-to- child HIV transmission (MTCT). However, pregnant women need to know that they are HIV-infected to take advantage of antiretroviral therapy, and many women do not know their HIV status. One-half of HIV-infected infants in the United States were bornto women who had not been tested for HIV or for whom the time of testing was not known. Although fewer than 400infants are infected perinatally in the United States each year, that number could be reduced even further through policies aimed at HIV testing during pregnancy.
The reasons toadopt such a policy are strong: the pathophysiology of perinatal transmission is clear, prophylaxis is effective and safe, and the intended beneficiaries of the intervention - babies - cannot protect themselves.
Morphological correlates of fire-induced tree mortality in a central Amazonian forest

Tree characteristics were recorded from 2829 standing trees in 24 0.25-ha terra firme forest plots in central Amazonia, 3 y after a surface fire had swept through the study area. Sixteen of the plots were within forest that burnt for the first time at the end of the 1997-98 El Niño (ENSO) event, and the remaining eight plots were within unburnt primary forest. In order to investigate the morphological correlates of tree mortality, we measured tree diameter at breast height (dbh) and bark thickness, and recorded burn height, bark roughness and the presence of latex, resin and buttress roots. Leaf litter depth was also recorded at the base of all trees in the unburnt forest. Using logistic regression models, tree mortality was best explained by the burn height, although dbh and the presence of buttresses were also important. Buttressed trees were associated with deeper leaf litter accumulation at their bases and higher char heights than trees without buttresses. Moreover, trees surviving the fire had significantly thicker bark than living trees in unburnt forest plots, indicating that thin-barked trees are more prone to selective mortality induced by heat stress. Latex did not appear to have had any significant effects on mortality, though resins were less abundant amongst the live trees in the burnt forest than in the unburnt controls. Levels of fire-mediated tree mortality in this study are compared with those in other Amazonian forest regions in light of historical factors affecting tree resistance to fires.
